La mayoría de los que trabajamos en urgencias nos apasiona nuestro trabajo. Nos hacemos adicto a esa pizca de adrenalina.
Pero, si ya como médico de familia nos toca bailar sobre la delgada línea de la incertidumbre, imaginaos como médico de familia en un servicio de urgencias extrahospitalarias, el baile sube de nivel y exige una mayor concentración.
No tuvimos un camino fácil, la carrera de medicina nos robó parte de nuestra juventud, el examen MIR nos ancló a una silla durante meses o algunos por años en búsqueda de la especialización deseada (o la mejor pagada); luego cuando ya parece que todo ha terminado a dar vueltas para buscar un puesto de trabajo estable, y siempre formándote, formándote y formándote.
Pues bien, en mi puesto de trabajo actual, el SUE Archena somos formadores por excelencia, en casi todas las guardias,, repasamos conceptos, técnicas y protocolos en urgencias, suplimos con nuestra inquietud algunas institucionalizadas carencias.
Hace unas semanas tuvimos uno de esos días en urgencias donde pacientes, virus y burocracia se pusieron de acuerdo para no darnos un minuto de tregua. En plena madrugada, cuando mi cuerpo por fin se desplomó sobre aquel destartalado sofá, y mis ojos cerraron las persianas, un telefonazo me puso de nuevo en alerta, el 112 nos activaba.
El particular sonido del móvil de urgencias, es una de esas cosas que no se te olvida fácilmente, somos como el perro de Pavlov respondemos a ese estímulo.
Pues bien, una noche fría y un equipo de soporte vital avanzado al completo, formado por un técnico en emergencias sanitarias, una habilidosa enfermera en urgencias y yo, rumbo a un aviso, motivo: probable muguet en la lengua. («le molesta la punta de la lengua»). [trágame tierra]
Este tipo de avisos, son esos que dinamitan cualquiera de tus pilares, son esos que te hacen cuestionar para que tantas horas de formación, actualizaciones y cursos, y si realmente merece la pena seguir subido a una ambulancia dando tumbos a estas horas de la madrugada.
Cuando llegamos, el paciente era un conocido por el servicio (no por el 112 que aparentemente no queda registro). Desde el interior de su cama, sin perder su calor intrínseco, nos relata que desde hace semanas tiene un zona más rugosa en la punta de la lengua y que aunque no le duele ni le impide la alimentación, le gustaría saber (a estas horas de la madrugada) que es, y por este motivo llamó al 112.
El aviso duró muy poco, lo solucioné rápido. No merecía la pena perder más tiempo del necesario, debemos estar libres por si surge cualquier emergencia. La llamada posterior a mis compañeros del 112 me la reservo, tampoco es el motivo de este post, reconozco sus limitaciones aunque hay salidas que nunca deberían producirse.
De regreso a la base, me queda temporalmente un sabor agridulce en la boca, son esos momentos que piensas «así no me gusta trabajar en urgencias», necesito un cambio. Por suerte algún compañero me saca una sonrisa y se me olvida rápidamente este tipo de avisos.
Todavía no llegó mi hora de abandonar la ambulancia, aunque parece que los años me pesan y algunas guardias también.

Lo que me extraña es que no verificaran antes de salir, el aviso.Me explico.En más de una ocasión por desgracia he tenido que llamar a urgencias.Una vez hecho el aviso, me llama el médico y me hace una serie de preguntas y si no lo considera necesario no acude.
Si no hay fiebre alta y otros signos alarmantes no acuden.
Me imagino que los que trabajan en el 112 obedecen o deben obedecer protocolos, pero en ocasiones, algunos triajes fallan y se deben revisar. Nosotros en la calle vemos lo que nos mandan, pero también es cierto que no vemos lo que nos quitan. Todo es mejorable y debemos trabajar para que así sea.
Creo que es una de las primeras, si no la primera vez que escribo por aquí.
Hace casi dos años me monté en una ambulancia por primera vez. Iba de prácticas, procurando ver, oír y callar hasta que me dejaban intervenir, cada vez dando un paso más. Desde entonces me quiero considerar una más de la «familia ambulanciera», intento entrar en una siempre que el tiempo me lo permite.
Sin embargo, la ley se ha puesto en mi contra. Con la nueva ley, mi formación especializada de carrera y postgrado no me permite entrar en una ambulancia. Porque no, no soy médico, enfermera o técnico. Soy psicóloga. Psicóloga de emergencias, para más datos, porque hice el postgrado en especialización en emergencias y catástrofes. Y aunque en la ambulancia en la que montaba siempre iba acompañada de los dos técnicos de rigor que formaban el soporte vital básico (y que gracias a mi formación no me he encontrado un caso en el que no supiera qué hacer a nivel sanitario), he tenido que dejar de subirme en mi querida ambulancia, en ocasiones en perjuicio del paciente.
En la Comunidad de Madrid, además, existe el equipo Psicosocial de Cruz Roja, un grupo de voluntarios que, sin ser psicólogos, hacen la «misma labor» que hago yo, haciendo ver a algunos, que por fortuna son cada vez menos, que los psicólogos, en realidad, no somos necesarios, basta con llamar a los voluntarios de cruz roja, que están de guardia siempre.
Lo que quiero decir con toda esta historia, es que entiendo que hay llamadas que no se deberían atender (espero que el rapapolvo al amigo de la lengua fuera serio), pero valorad que os llamen, aunque sea para una minucia, porque significa que reconocen vuestro trabajo.

Siempre que sale un equipo Psicosocial de cruz roja, salen mínimo un psicólogo titulado y un conductor.
Hoy por hoy la comunidad de Madrid cuenta con psicólogos en samur social y emergencia social..
Pero la activación de estos es complicado y por tanto hay sucesos en los que se requiere una intervención psicosocial que nunca llega..
Es aquí donde entra Cruz Roja. Todo lo que sea Comunidad de Madrid (salvo ayuntamiento de Madrid vía pública que samur tiene un psicólogo de guardia 24 h) cruz roja es requerida por medicos del summa112.
Cruz roja como norma general tiene equipos Psicosociales operativos los fines de semana, y entresemana cuando existe alguna necesidad contacta con sus voluntarios y destina un equipo en alrededor de 1 hora y media.
Por esta demora no siempre continuan la activación de cruz roja.. Porque no están como te piensas operativos siempre…
El problema no es el teleoperador,(aunque algunos tienen su miga)es el médico regulador.Sus limitaciones operativas son las mismas que tienen en todas las comunidades, no es excusa. Hola, buenos días, me llamo Raquel, vivo en Archena y soy auxiliar de enfermeria, no es d sorprender este tipo de cuestiones y preguntas a altas horas de la madrugada, he trabajado durante 6 años en la puerta de urgencias d la Arrixaca y 2 en la del Morales Meseguer y se a que de que hablas perfectamente. Pienso q en realidad no hay q juzgar ni al telefonista no al 112 ni a nadie, lo q hay q juzgar es la poca información de los ciudadanos de q es o no es una urgencia vital. Hay muy poca información sobre eso y menos aún en personas mayores. Habría q educar en este tema al pueblo. Un saludo
Yo trabajo en otro SUAP de la región, y avisos como el tuyo, desgraciadamente también nos pasan. La ciudadanía no tiene la culpa, sino este sistema. Dónde está la educación sanitaria? La publicidad solo dice, llamar al 112. La administración es la responsable de todo el caos que vivimos en las guardias y la política de educación sanitaria debe ser prioritaria. Además de que la atención primaria cumpla su papel educador.
Es evidente que todos tenemos responsabilidad, la población, los profesionales y los servicios de salud / gerencias. Debemos detectar y corregir los defectos del sistema, y minimizar este tipo de avisos que no hacen bien ni al paciente ni al profesional, y menos a un sistema que necesita mantener unos servicios con unos recursos económicos limitados. Llegué aquí por casualidad.Yo también soy de las que paso muchas horas en ambulancia por este tipo de avisos,que en un principio también me pueden molestar.Luego intento reflexionar el motivo que puede tener ese paciente para llamarnos a altas horas por esa nimiedad.Siempre hay una razón,no me cabe duda.Ya sea su soledad,su preocupación o angustia.Estamos para ello,para algo más que para paliar ese muguet en lengua.

He de decir que en el 112 trabajan también médicos, que perdieron parte de su adolescencia estudiando, como usted, y que se forman cada día por trabajar mejor. No subestime el trabajo de muchos, por la vagancia de pocos.
Por supuesto que trabajan médicos, y no sólo ellos estudiaron y se prepararon, también los compañeros enfermeros, tes, teleoperadores que intentan hacer su trabajo lo mejor posible. Yo no subestimo ni el trabajo ni la profesión. Pero hay aspectos que se deben mejorar, y tener la capacidad de autocrítica para poder avanzar es una necesidad. Porque está historia, no trata del 112, sino de los profesionales que a pie de calle sufrimos este tipo de aviso que mellan nuestra motivación, y por los comentario y difusión no somos pocos.
Las historias Con Tinta de Médico plantean preguntas pero en ocasiones la participación de profesionales y pacientes nos dan las respuestas.
Solo hay que tener la capacidad de poder verlas.

He llegado aquí de casualidad, a través de una amiga que es médico y hace dos noches le tocó el turno de urgencias y, para variar, hubo gente que fue al hospital por tonterías.
Vale que hay que informar a los ciudadanos, vale que no siempre se dispone de todos los recursos que nos gustaría, vale que el personal sanitario a veces no da más a basto… Pero de verdad, hay gente de bofetón; que con un poco de sentido común que se tenga, te queda claro que no deberías hacer ir a una ambulancia a tu casa porque te molesta la punta de la lengua (por seguir con vuestro ejemplo). Que hay cosas importantes de verdad, y todos deberíamos tener claro que si nuestro tiempo es valioso, el del personal sanitario lo es mucho más.
A mí es que, sinceramente, me parece una falta de respeto que os hagan perder el tiempo con tonterías. Y a veces me da hasta cargo de conciencia pensar esto, pero si nos cobraran por usar el servicio de urgencias, estoy segura de que mucha gente se lo pensaría antes de llamaros o haceros desplazaros para cosas que al final resulta que no eran tan urgentes…
Las típicas picaceras, escozores y demás capulladas. En ocasiones he fantaseado con enviar a que les atienda «el tío la vara», pero hay que ser políticamente correcto y tragar; no sólo en esto, si no en tantos anacronismos como se producen en éste medio. Hace mucho tiempo, que nos lo teníamos que haber hecho mirar, así nos va el pelo. Y que dure.
lo que hay no es desinformación sino muy poca vergüenza, un supino egoismo por parte de algunos usuarios pues los recursos que hay son de todos nosotros y para todos nosotros y hay que optimizarlos ..y hay que cuidarlos. Con la sanidad que yo ejerzo, que yo pago, que yo disfruto…no se puede permitir ese abuso. Se debe penalizar el mal uso a sabiendas o bien, respaldar a quién rechace cursar semejante atención médica.
perdonar mi ignorancia, pero podrian decirme que cantidad de casos digamos 10% de las actuaciones serian casos como estos?.
la pregunta tiene una logicà pero no quiero molestar con toda la historia.

No estoy tan desinformada respecto a Cruz Roja (sobre SAMUR social sí que no tengo ni idea la verdad, aunque tengo entendido que no hacen emergencia, cubren otro tipo de servicios, pero vamos, que no sé nada de ellos aparte de que lo gestiona Grupo 5), he formado parte del equipo durante mis prácticas, y al referirme con que están siempre disponibles me refiero a que si ocurre algo entre semana, sea la hora que sea, movilizan a los voluntarios por si se pudiera cubrir el servicio.
El que en el equipo de Cruz Roja vaya un psicólogo es casi testimonial, porque todos hacen la misma labor que el psicólogo, sin importar que sean otra cosa, la única diferencia es que en el chaleco del psicólogo pone además «Jefe de equipo», pero he estado en intervenciones donde el peso de la intervención la ha llevado un sociólogo o una administrativa, que por mucha experiencia que tengan, no llegan a tener la formación teórica de entre 5 y 6 años de un psicólogo con la especialidad. A eso es a lo que me refería en el comentario con que no se nos reconoce, porque siempre que haya gente sin la carrera y la especialidad, que con un curso de unas pocas horas (por muy útil que sea siempre se va a quedar corto) va a hacer lo mismo que yo, que tengo 6 años de formación, no conseguiremos un puesto reconocido en la emergencia, aunque haya compañeros que a título personal sí que reconozcan que han echado en falta, el algunos momentos, a psicólogos en las unidades.
Ah…Las bondades de los sistemas de sanidad universales pagados por algunos y disfrutados por todos…¿No es motivador saber que gracias a tener los sueldos mas bajos de la medicina de Europa Occidental todos los hiperfrecuentadores, toxicómanos e hipocondriacos tienen acceso libre e infinito al sistema sanitario? 🙂

Have you ever thought about adding a little bit more than just your articles? I mean, what you say is fundamental and all. However think of if you added some great photos or video clips to give your posts more, «pop»!
