Ya no visto de blanco. Mi piel alberga nuevas cicatrices y mi rostro olvidó aquel rasurado perfecto, pero recuerdo ese día como si fuera ayer.
La visita de la mañana iba a comenzar, quien haya conocido la medicina por Sudamérica puede dar fe de lo crueles que pueden llegar a ser con el médico interno y residente aquellas sesiones a pie de cama, delante del paciente, con preguntas y más preguntas hasta hacerte ver lo poco que sabes, una forma dictatorial de aprender que creo que todavía debe mejorar.
Da igual como haya sido tu turno de noche, no importa si tienes guardias un día si y otro no, porque a la mañana siguiente tienes que defender la historia clínica de tu paciente e intentar escapar sin muchos revolcones de la temible visita.
Personalmente no me puedo quejar, a pesar de ser «el médico español» durante mi paso por el internado en Bolivia, mi espíritu inconformista me ayudó a salir airoso de muchas batallas, algunas que con el paso del tiempo se han convertido en divertidas anécdotas que algún día contaré.
En esa época todavía creía que una persona sola podía cambiar el mundo y era feliz haciendo lo que hacía, no me importaba el MIR, no me importaba en que hospital haría la residencia, la habitación donde dormía, ni cuanto dinero podía ganar ejerciendo la medicina privada, solo disfrutaba de la medicina, del descubrimiento de enfermedades y de mis pacientes.
Las noches en vela merecían la pena, no solían despertarme por un caprichoso dolor de garganta de 4 días de evolución que no cedía con paracetamol, las urgencias se respetaban.
Esa mañana en concreto, tenía que presentar los nuevos ingresos en el Hospital de Montero (Santa Cruz-Bolivia), mi descanso nocturno brilló por su ausencia, pero al pasar por una habitación no pude resistir la tentación de visitar a uno de mis pacientes, aquel que el día anterior llegó apático, con ojos hundidos y mucosas secas, deshidratado por una simple diarrea que casi le cuesta la vida. No dudé en aprovechar la oportunidad para enseñarle a una madre con prisas, como tenía que rehidratar a su hijo, ya que ellos vivían alejados del puesto médico y a veces sus obligaciones les impedían desplazarse cuando surgía un problema de salud.
La rehidratación IV no siempre está disponible en algunos rincones de este trópico, por eso, para evitar deshidrataciones severas se usan las sales de rehidratación oral (OMS) o la preparación del suero oral casero, además de poner énfasis en el lavado de manos, medidas básicas que con un poco de paciencia pueden salvar vidas.
Todavía recuerdo la cara de la enfermera, y como su mirada de complicidad me alertaba de que me estaba retrasando. Me insistía que la inquisidora visita ya había comenzado, y que como llegara tarde la iban a tomar otra vez conmigo. A pesar de tener presente un «me la voy a cargar» en mi mente, a veces nos toca desobedecer y hacerle caso a nuestros instintos, porque ¿qué sería del mundo si todos siempre obedeciéramos?.
Por supuesto llegué tarde, no suelo ser puntual. Me excusé y acepté el revolcón con orgullo, pero nadie pudo borrarme la sonrisa de la cara, esa sonrisa que se te queda cuando estas orgulloso de tu trabajo, una sonrisa que en ocasiones echo en menos.
Y es que por aquellos años todavía tenía fe ciega en la medicina humanitaria, en las políticas de salud de los gobiernos y en una desinteresada cooperación sanitaria.
Ahora que mi barba comienza a teñirse de blanco y que mi experiencia en atención primaria y urgencias ya suma en mi curriculum, todavía le sigo dando vueltas a la mejor forma de cooperar con Latinoamérica, y en especial con Bolivia.
Habrá que sacar de nueva las botas del armario y cargar la mochila de nuevos proyectos.
Porque aunque mis obligaciones y un «si te vas, te la cargas» me dicen que ahora no es el mejor momento, ya me empieza a picar de nuevo el gusanillo de la cooperación sanitaria ¿y quién es capaz de decirle no a un sueño?

Why? Because I was happy to discover how well-designed and fun to use the underappreciated (and widely mocked) Zunes are.

If you’re still on the fence: grab your favorite earphones, head down to a Best Buy and ask to plug them into a Zune then an iPod and see which one sounds better to you, and which interface makes you smile more. Then you’ll know which is right for you.
This is getting a bit more subjective, but I much prefer the Zune Marketplace. The interface is colorful, has more flair, and some cool features like ‘Mixview’ that let you quickly see related albums, songs, or other users related to what you’re listening to. Clicking on one of those will center on that item, and another set of «neighbors» will come into view, allowing you to navigate around exploring by similar artists, songs, or users. Speaking of users, the Zune «Social» is also great fun, letting you find others with shared tastes and becoming friends with them. You then can listen to a playlist created based on an amalgamation of what all your friends are listening to, which is also enjoyable. Those concerned with privacy will be relieved to know you can prevent the public from seeing your personal listening habits if you so choose.

When I initially commented I clicked the «Notify me when new comments are added» checkbox and now each time a comment is added I get several e-mails with the same comment. Is there any way you can remove people from that service? Bless you!

Apple now has Rhapsody as an app, which is a great start, but it is currently hampered by the inability to store locally on your iPod, and has a dismal 64kbps bit rate. If this changes, then it will somewhat negate this advantage for the Zune, but the 10 songs per month will still be a big plus in Zune Pass’ favor.

The Zune concentrates on being a Portable Media Player. Not a web browser. Not a game machine. Maybe in the future it’ll do even better in those areas, but for now it’s a fantastic way to organize and listen to your music and videos, and is without peer in that regard. The iPod’s strengths are its web browsing and apps. If those sound more compelling, perhaps it is your best choice.
At this time it seems like Movable Type is the top blogging platform out there right now. (from what I’ve read) Is that what you are using on your blog?

Hands down, Apple’s app store wins by a mile. It’s a huge selection of all sorts of apps vs a rather sad selection of a handful for Zune. Microsoft has plans, especially in the realm of games, but I’m not sure I’d want to bet on the future if this aspect is important to you. The iPod is a much better choice in that case.

This is getting a bit more subjective, but I much prefer the Zune Marketplace. The interface is colorful, has more flair, and some cool features like ‘Mixview’ that let you quickly see related albums, songs, or other users related to what you’re listening to. Clicking on one of those will center on that item, and another set of «neighbors» will come into view, allowing you to navigate around exploring by similar artists, songs, or users. Speaking of users, the Zune «Social» is also great fun, letting you find others with shared tastes and becoming friends with them. You then can listen to a playlist created based on an amalgamation of what all your friends are listening to, which is also enjoyable. Those concerned with privacy will be relieved to know you can prevent the public from seeing your personal listening habits if you so choose.

Apple now has Rhapsody as an app, which is a great start, but it is currently hampered by the inability to store locally on your iPod, and has a dismal 64kbps bit rate. If this changes, then it will somewhat negate this advantage for the Zune, but the 10 songs per month will still be a big plus in Zune Pass’ favor.

The new Zune browser is surprisingly good, but not as good as the iPod’s. It works well, but isn’t as fast as Safari, and has a clunkier interface. If you occasionally plan on using the web browser that’s not an issue, but if you’re planning to browse the web alot from your PMP then the iPod’s larger screen and better browser may be important.

Hands down, Apple’s app store wins by a mile. It’s a huge selection of all sorts of apps vs a rather sad selection of a handful for Zune. Microsoft has plans, especially in the realm of games, but I’m not sure I’d want to bet on the future if this aspect is important to you. The iPod is a much better choice in that case.

Today, I went to the beach with my children. I found a sea shell and gave it to my 4 year old daughter and said «You can hear the ocean if you put this to your ear.» She put the shell to her ear and screamed. Hello, I think your website might be having browser compatibility issues. When I look at your blog in Opera, it looks fine but when opening in Internet Explorer, it has some overlapping. I just wanted to give you a quick heads up! Other then that, awesome blog!

When I initially commented I clicked the «Notify me when new comments are added» checkbox and now each time a comment is added I get four emails with the same comment. Is there any way you can remove me from that service? Cheers!
