Ayer saliente de guardia. En mitad de mi descanso me desperté pensando que tal vez tendría que haber usado dopamina en un paciente al que soñé atender hace ya algunas lunas. Después de unos minutos me acordé que nuestra unidad por una incomprensible mezcla de desidia y burocracia no está equipada con ese y otros fármacos esenciales. Finalmente ocho palabras golpearon mis cimientos, «solo soy un Médico de Familia en Urgencias».
No soy un médico intensivista.
No soy de UCI.
No veo al paciente como una enfermedad.
No tengo en mi mente tatuado cada detalle de una guía farmacológica.
No dedico todo mi tiempo libre a leer libros de Urgencias.
Ni tengo el corazón cubierto por una escarcha de hielo.
Sólo soy un Médico de Familia en un servicio de Urgencias, y por esto, irremediablemente siempre veo a mi paciente como un todo.
Veo a un ser humano con nombres y apellidos. Con familia, trabajo y problemas.
Me fijo en como se viste, como huele, como tiene decorada su habitación o si hay restos de comida por el suelo.
Las fotos del salón me cuentan su historia. Los acompañantes me la confirman.
Cuando me enseñan su tratamiento, observo la bolsa con las cajas de los medicamentos, las cuales a veces duplicadas o sucias me trasmiten más información que echar un vistazo en una historia clínica.
Escucho su mirada y veo sus palabras.
Me especialicé como médico de familia y trabajo en Urgencias, no soy un especialista en urgencias que trabaja en atención primaria.
Soy el responsable docente de los residentes de Medicina de Familia que rotan por mi servicio de urgencias extrahospitalarias, y lo considero el lugar idóneo (aunque muy mejorable) para que un médico de familia pueda trabajar en el área de Urgencias. Les intento enseñar aquello que en mi especialidad no tuve la oportunidad de aprender y el lado más humano de la medicina.
La lucha de las sociedades médicas por conseguir una especialidad o área de capacitación en Urgencias nunca me interesó. Veo absurdas y erróneas algunas amenazas. Estoy cansado de escuchar palabras vacías en congresos llenos. Harto de la mediocridad de algunos sillones y de incompetentes cargos puestos a dedo. De cualquier modo ahora sé que es necesario cambiar nuestro periodo de formación. Sé que es imprescindible darle un giro. La formación en Urgencias para los médicos de familia en España debe reorganizarse. Si somos una de las especialidades que aporta un mayor número de profesionales que trabajan en esta área, el programa de la especialidad del MIR de Familia tiene que mejorar. Tal vez sea el momento de replantear un cambio o por lo menos un acercamiento de intransigentes posturas que solo perjudican a los profesionales que trabajamos en Urgencias.
Y es que estar a pie de calle, en un consultorio perdido del sistema, en una ambulancia sin recursos o en interminables jornadas en la puerta de urgencias de un hospital está muy alejado de algunos cómodos sillones donde se toman malas decisiones.
Tal vez, no sea de los mejores.
Tal vez, no sea de los que cumplen absurdas reglas.
Tal vez, solo estoy aquí por un capricho del destino.
Pero os aseguro que ya no soy de los que callan.
Ni de los que giran su cabeza hacia otro lado evitando solucionar un problema.
Solo soy un médico de familia en un servicio de urgencias.

José, como siempre acertado mi estimado amigo, yo sin duda apoyo la necesidad de mejorar la formación médica en general, de la mano de estimular un cambio de actitud en los médicos estudiantes y residentes. Idealmente en todos!!. ¿Tarea difícil?… ¡claro!, lo más sencillo es y será perpetuar situaciones y conductas y no remover rincones incómodos en donde diversos intereses moran desde la eternidad. No obstante, como un punto de vista personal y sin intereses de por medio, ( rayando la utopía), yo apostaría por la creación de la especialidad de Urgencias. Es bien claro que muchos elegimos especialidad con conocimiento de ella y sensación de pertenencia a todo lo que ella implica, pero, también es cierto y no lo digo de forma peyorativa, que otros no lo hacen así y después, dando lo mejor de sí, se dedican a la atención de urgencias. Pues bien, cada especialidad te condiciona, la especialidad no sólo te brinda la oportunidad de «dominar» una pequeña parte de la Medicina, a cambio acentúa ciertas características de personalidad y forma de pensar, por lo que, nacer como especialista en un servicio de Urgencias, aprender de él y dejarte moldear por las muchas especiales características de estos servicios, haría alguna diferencia. Ya que si bien es cierto que los seres humanos no podemos ser reducidos meramente a órganos y sistemas para su atención, a veces en el momento preciso, se requiere salvar un corazón, abrir una tráquea o suturar una herida sin saber salvo el nombre extraído de una identificación, lo cual generalmente, no disminuye el interés por la persona, simplemente te resta la carga afectiva de la relación médico paciente.
Reitero mi admiración y respeto por todos mis compañeros que atienden los servicios de urgencias dando lo mejor de sí a cada momento, queda claro que sin su trabajo las cosas serían diferentes y que bajo el uniforme, ya no existen las especialidades, somos todos iguales con un interés común.

Buenas José. Me ha encantado tu post. Hace unos años escribí una editorial en una revista que se titulaba: «Somos Medicos de Familia» y en el defendía que los médicos de familia éramos los médicos con una formación mas adecuada para las urgencias.
Pienso como tu que hay que mejorar el Plan de nuestra Especialidad adecuándolo a los nuevos tiempos y también que habrá que ayudar a mejorar las competencias de los médicos de familia.
Pero algo de lo que nunca debemos renegar los Médicos de Familia es de nuestra especialidad, trabajemos donde trabajemos (Atencion primaria, urgencias extrahospitalarias, urgencias hospitalarias, mutuas….). Somos médicos de familia y siempre lo seremos.

Desde otra realidad (soy argentina y trabajo en un pequeño hospital) entiendo lo que dices.
En general en Argentina las guardias (servicios de urgencias) las realizan médicos recién recibidos que todavía no tienen experiencia. En las grandes ciudades ya existe la especialidad pero no en el interior del país.
Lamentablemente la guardia es, muchas veces, la puerta de entrada al sistema y no siempre los médicos que están a cargo de ella están capacitados.

Hola, soy enfermera de urgencias, y aunque me ha gustado en general tu escrito, me ha dolido bastante las referencias al personal que trabaja en urgencias. Como en todos los lugares y profesiones, habrá personas de todas las condiciones, y he conocido también alguna indeseable, pero no tenemos el corazón con escarcha de hielo, sacamos fuerza y valor de dónde a veces no sabíamos que existía, y luchamos por ganar la batalla y salvar al paciente, y también lloramos y nos cansamos de ver el dolor y el sufrimiento, el crónico y el que llega por sorpresa para arrebatártelo todo. Acompañamos al paciente y a la familia, le damos la mano y buscamos su bienestar y confort dentro de las posibilidades de las que disponemos. Y en toda esa lucha también hace falta conocer al dedillo la farmacología y un poco de todo, porque muchas veces no tienes al lado a nadie y de tì depende todo. Es un trabajo poco agradecido, pero para la mayoría de nosotros nos reconforta el saber que damos lo mejor de nosotros tanto en un resfriado como ante una parada. Y muchas veces conoces al paciente profundamente debido al roce cercano de una observación de urgencias, de una sala de críticos o en una simple cura o técnica.
Hola José, escribo por alusiones para decirte que estando de acuerdo con el final de este post, no puedo sino disentir con muchas de las cosas que en el mencionas.
Para empezar, decirte que soy intensivista, que en este mismo momento que estoy escribiendo estas palabras desde mi sofa, estoy recién llegado de una guardia de 24h en mi unidad de cuidados críticos tras haber estado a cargo de más de 20 pacientes de estas caracteristicas. Decirte que el corazón helado y de escarcha del cual nos presupones, se ha derretido al inicio de esta guardia (si no lo estaba ya), debido a que por encima de esta enfermedad que presupones que solo somos capaces de ver, he tenido que retirar las terapias de soporte vital a un paciente que cumplia su 18º día de ingreso en UCI por una neumonía que no se resolvía, resultando ser una enfermedad diferente, sin posibilidad terapeutica alguna. Decirte que durante esos 18 días, además de aplicar VMNI con IPAP/EPAP variable, decidir que soporte vasotropo ponerle y a cuantos mcg/kg/min, he cometido la desfachatez de sentarme todos los días a hablar con su mujer y con el mismo, hasta que no quedo más remedio que aplicar sedoanalgesia y aislar la vía aérea mediante TOT y VM (como verás hasta ahora soy el intensivista robotico y lleno de siglas, típico y topico estereotipo). Como te decía he cometido la desfachatez e ignominia de preocuparme por como se sentían, tanto el enfermo con su mujer, de hablar de sus miedos y preocupaciones, de cual era su profesión y su unidad familiar e incluso de conocer a sus dos hijos pequeños, de 10 y 12 años y estar presente (de motu propio y por petición de su mujer, en el momento de la despedida definitiva, mientras ella le leía una carta realizada por sus hijos, despidiéndose de su padre). He llorado cada lagrima por ese paciente y por muchos otros junto a madres, hijos, mujeres y demás familia con el corazón tan encogido como ellos, libre de esa escarcha que dices que tenemos. He tratado de todas las maneras posibles de solucionar la enfermedad, la inestabilidad hemodinamica y los problemas fisiopatológicos del enfermo crítico, pero también he sido psicólogo, oyente, confidente y amigo de muchos de mis pacientes y sus familias, en sus buenos y malos momentos. Cada sonrisa que les he arrancado ha sido para mi un éxito comparable como conseguir la estabilidad hemodinámica en él, el colocar con éxito un catéter de arteria pulmonar o reanimar una parada cardiorrespiratoria.
Una vez dicho esto, estoy de acuerdo con la necesidad de crear una especialidad de urgencias, como han comentado algunos otros compañeros que escriben, motivados por tus palabras , cada uno por sus motivos a los cuales no voy a entrar para desatar la polémica.
Sin más me despido, no me hagas mucho caso, ya que soy un intensivista saliente de guardia que trabaja en una UCI, abrumado por la fatalidad e irreversibilidad de la muerte y en algunas ocasiones por un medico de familia que trabaja en urgencias.

No soy ni médico ni enfermera ni tengo ninguna titulación sanitaria. Soy paciente, una de esas que rompen los esquemas de todas los médicos y a la que no han solido tratar muy bien porque no comprendían que es lo que podía estar pasándome, así que «chapeau» por tu humanidad y comprensión hacia las personas, que creo que siempre tiene que ir por delante de la propia medicina, ya que todavía no se tienen todas las respuestas para todos los problemas. Ya trabajé en primaria y era médico domiciliario, ahora internista. Sigo igual que usted pensando cuando recapacitaremos para que los médicos en formación vean el paciente como todo. Tenemos que hacer que el SES cambie porque haciendo lo mismo seguiremos igual.
Estimados compañeros de UCI/Cuidados Críticos, como mencioné trabajo en las Urgencias Extrahospitalarias y fui residente de Medicina de Familia en Hospital Virgen de la Arrixaca (Murcia), donde hay un servicio de UCI en el cual hacían sus guardias compañeros de residencia, y que por el que lamentablemente nunca rotamos debido a que no está incluido inexplicablemente en nuestro programa de la residencia. Y te digo esto, porque creo que mis palabras fueron malentendidas, y lo lamento. Estoy orgulloso de los profesionales que trabajan en servicios como el tuyo, y aunque yo personalmente no podría, sé que la humanidad no la da pertenecer a un servicio sino las personas que trabajan en él. Recuerdo el alivio que me daba veros por la puerta de Urgencias cuando os necesitaba con mi pijama de residente.
Por eso, cuando escribo «No soy un Médico Intensivista. (Punto y aparte) No soy de UCI. (Punto y aparte), no veo……Cada frase solo significa lo que pone. No hablo que los médicos de UCI o los Intensivistas tienen el corazón de escarcha, por supuesto, solo que yo no lo tengo. Tal vez mi estilo, deja la puerta abierta para todo tipo de interpretaciones, lo lamento.
Yo también estoy hoy saliente de guardia, y no merece la pena contar todo lo que hice. Hoy toca asimilar la difusión de este post, y valorar la necesidad que tenemos todo el colectivo que trabajamos en estos servicios por cambiar ya algunas cosas. Tal vez, los que ocupan sillones en Sociedades, Comisiones o Ministerios son los que debería estar rompiéndose hoy la cabeza. Un abrazo.

Creo que la esencia de lo que intentas transmitir es la pésima organización sanitaria y la falta de formación adecuada y real para algunas especialidades pero sólo corregirte en algo….los medicos intensivistas, los de UCI, no sólo miramos la enfermedad….la dureza del trabajo con pacientes críticos es igual para el que lo atiende en la calle completamente desvalido de medios, como para el que lo atiende en un hospital con mejores características….. En esta lucha no estamos unos contra otros, sino todos juntos, medicos de familia que trabajan duramente en los servicios de urgencias como intensivistas, anestesistas, cirujanos…. Y tenemos que estar unidos para luchar contra el mal que nos acecha en la sanidad pública. Tenemos que luchar por mantener las cosas buenas de nuestra profesión y mejorar todas las que nos empequeñecen…. Mucho ánimo para seguir con tu día a día que los que estamos en la UCi de puertas para adentro seguiremos luchando por continuar con el buen trabajo que vosotros comenzáis!

Me siento orgullosa de los médicos españoles. Lamentablemente muy mal pagados. Es y debería ser una obligación de todos los gobiernos dotar de medios a esta clase de médicos que ademàs dad grandes profesionales practican la bonhomía.

Es la cruda realidad.A medida que iba leyendo iban pasando por mi cabeza multitud de imagenes creadas con mi mente recreando las historias y batallas que mi mujer me cuenta,pues ella también es sólo un médico de familia en una puerta de urgencias y el sistema y la falta de recursos y el abuso de de.las urgencias por parte de otros te hacen ver patologías donde antes veías personas con sus problemas,actuando con la espalda contra la pared para que no te denuncien por explicar al usuario que su problema no es una urgencia(aunque para el si.lo sea),o tantas otras cosas.

TENGO MAL RECUERDO DE LOS MÉDICOS DE URGENCIAS DE UN HOSPITAL CONCRETO QUE NO SUPIERON VER QUE MI MARIDO SE ESTABA DESANGRANDO, CON LA AORTA ROTA.LO MANDARON EN BUSCA DE UN DIAGNÓSTICO A OTRO HOSPITAL A 90KM DE DISTANCIA, LUEGO A OTRO.ERA DOMINGO POR LA NOCHE.MURIÓ EN EL QUIRÓFANO A LA MEDIA HORA DE ENTRAR.PERO SE PERDIERON 7 HORAS QUE ESTUVO CON VIDA.NO LLAMARON UN ELICÓPTERO QUE HAY PARA DERIVAR ENFERMOS AL HOSPITAL CENTRAL QUE ES A DONDE DEBIA IR,ADEMÁS NOS «TRANQUILIZABAN» DICIENDO VAYAN DESPACIO.ELLOS LO ACOMPAÑARON EN AMBULANCIA PERO PARA QUÉ? Estimado colega, buenísima reflexión…y si en España todo es mejorable, qué le podría decir de mi país el Perú donde todavía hay mucho por hacer en nuestra especialidad y en la mejora de nuestros servicios y del sistema…trabajo en un hospital del estado en triaje de emergencia y atención de urgencias y con sus palabras ha dado en el clavo en la causa de muchos problemas que aquejan en nuestros sistemas de salud… un saludo desde Lima, Perú.

A MENUDO DEBEMOS HACER DE TRIPAS CORAZÓN A CADA PASO Y CONTROLAR EL MIEDO. ES NORMAL SENTIRLO PERO NO DEBEMOS PERMITIR NUNCA JAMÁS QUE NOS BLOQUEE. EL MIEDO NOS MANTIENE ALERTA Y NOS ALEJA DEL EXCESO DE CONFIANZA QUE A MENUDO ES LA PRINCIPAL FUENTE DE ERRORES.
CUANDO ATENDAMOS A ALGUIEN DEBEMOS TENER MUY PRESENTE ESA PERSONA QUE VEMOS TIENE PADRES, HIJOS, ESPOSO, AMIGOS, Y TODA UNA VIDA COMO LA NUESTRA. TENEMOS QUE ESCUCHAR, INFORMAR, NEGOCIAR PERO SIEMPRE CONSULTANDO CON EL PACIENTE SI ESTÁ DE ACUERDO CON NUESTRAS DECISIONES. PREGUNTARLE LAS CUATRO PREGUNTAS HIPOCRÁTICAS, ESCUCHARLE CON ATENCIÓN, HABLARLE CON RESPETO Y CARIÑO. ESTAR SIEMPRE ABIERTO DE MENTE POR SI UN DATO NUEVO NOS CAMBIA EL JUICIO CLÍNICO Y EL PROTOCOLO TERAPEUTICO.
HAY QUE TERER UN TRATO CORRECTO, DEBEMOS CONTENERNOS PERFECTAMENTE ANTE PROVOCACIONES, INSULTOS Y AMENAZAS PERO TAMBIEN DEBEMOS MANTENERNOS UN TANTO DISTANTES ANTE PERSONAS MUY AMIGAS Y FAMILIARES «PARA NO DISTRAERNOS NI SALIRNOS DEL TEMA».

Me he sentido muy identificada. Muchas gracias por comunicar los sentimientos de un médico de familia en urgencias de atención primaria. Tratar al paciente con sensibilidad y en todos sus aspectos es vital y manejar la incertidumbre del médico de familia en un consultorio local es una carga que se lleva siempre en los días siguientes a la guardia. Muy buenas! Que una reflexión genere tantos comentarios es un lujo, Jose Manuel. Me encanta. A mí me ha resonado lo de «palabras vacías en congresos llenos» , y creo que ante eso lo mejor es intentar expresarse como lo habéis hecho aquí . Gracias. Quiero alabar la sensibilidad del medico de UCI, y la valentía de la señora que perdió a su marido. Ella Sí que lo tiene difícil. ..tiene que aceptar la fatalidad de una disección ártica de su marido, y además que los médicos no la sospecharan. Te diré que yo también soy médico, y no siempre es fácil de sospechar esta patología, pero te pido perdón en nombre de mis compañeros es por no acompañarte y no arropar tras el desenlace, probablemente ellos también estarían hundidos. My developer is trying to persuade me to move to .net from PHP. I have always disliked the idea because of the costs. But he’s tryiong none the less.
